Dear Patient,

The use of the (non-) medical services of our practice is subject to the following conditions, unless we agree otherwise in writing:

1. When using social insurance services, you must present your E-Card at each visit to our practice, but at least at the first visit in each quarter, upon request.

2. The conclusion of an individual treatment contract is made through factual actions (your registration in the practice or the treatment by us).

3. Insofar as the treatments performed on you are not paid for by your health insurance, the fees and cost reimbursements incurred for this, as well as other charges, are due upon receipt of the invoice. You will be informed of this circumstance by Dr. Huber or his staff before such treatment.

4. Exclusion from insurance coverage: Various services provided in our practice, such as sexual medical consultation, private practice, sperm analysis, urodynamics (bladder function measurement), pelvic floor training, and vasectomy are private services for which no reimbursement by the health insurance is possible. If you have private health insurance for such services, we cannot guarantee the amount and extent of reimbursement by the private health insurance.

5. We assume no liability or other warranty for any items you bring into our practice.

6. Each patient receives a keycard at reception to facilitate the practice process. This keycard must be returned upon check-out. If the keycard is not returned, we reserve the right to charge a replacement fee of 30 euros.

7. Appointments: It is important to us that all patients in our practice have the shortest possible waiting times and that as many patients as possible can be treated during practice hours. Therefore, it is necessary that you keep agreed appointments or cancel them in a timely manner. If you do not cancel in writing using the email address storno@urologenzentrum.at at least 24 hours before an appointment, we may charge the incurred expense reimbursement. We also accept telephone cancellations, but we recommend a written cancellation for your own proof. Even if you do not cancel a private appointment (i.e., not for insurance services, especially the services mentioned above) at least 24 hours in advance, we are entitled to charge an expense reimbursement. An appointment for a vasectomy must be canceled at least 1 week before your appointment in the aforementioned manner, otherwise an expense reimbursement will be charged. In any case, if a vasectomy is canceled, €150 will be due for the consultation and premedication. If you postpone your vasectomy appointment more than twice, we are also entitled to charge an expense reimbursement. 

8. Appointment scheduling and report inquiry: You declare that if you use cgm-life.at for appointment scheduling, you agree that the data you enter there will be stored and used for the purpose of appointment scheduling and preparation. You can revoke this consent at any time.

9. We point out that reports sent to the practice without prior request by us or a related patient conversation will not be reviewed by us. Furthermore, patients are obliged to independently schedule an appointment for report discussion.

10. I agree that after each visit to the practice, I will receive an email informing me of the opportunity to rate this visit. I can revoke this consent at any time.

11. You declare that you agree to the automated (electronic) processing of your data (including health data) by Dr. Erik HUBER, Dr. Erik Randall Huber & Partner Specialists in Urology OG, and Dr. Erik Huber & Partner Specialist in Urology and General Medicine GmbH, particularly for the purpose of appointment scheduling and management as well as administration (including billing).

12. Video surveillance takes place in our practice to ensure the safety of our patients and staff. The recordings are made out of legitimate interest in accordance with legal regulations and are used exclusively for security purposes. Access to the recordings is only granted in specific cases and exclusively by authorized persons, in compliance with data protection guidelines.
